Introduction
Three HEALTH UTILITIES INDEX(R) systems have been developed: HUI Mark 1 (HUI1); HUI Mark 2 (HUI2); and HUI Mark 3 (HUI3). This document describes procedures associated with the two most recent of the systems, HUI2 and HUI3, and standard HUI 15-item questionnaires for self-completion. For a summary of the HUI measurement systems in "Frequently asked questions" format, including information about measurement properties (i.e., validity, reliability and responsiveness) and clinically important differences, see Horsman et al (2003) available free of charge from the publisher's web-site (http://www.hqlo.com/content/1/1/54/). HUI-related reference information, including "HUI Reference Scores" for various general population groups (i.e., population norm scores), is presented on the Health Utilities Inc. website (http://www.healthutilities.com/). The HUI2 health status classification system (Feeny et al 1992, Feeny et al 1995) is presented in Appendix A (page 28) and the HUI3 health status classification system (Feeny et al 1995, Feeny et al 1996) is presented in Appendix B (page 31).
